ที่มาและความสำคัญ

การแปรผันทางกายวิภาคของกระดูก clivus ซึ่งเป็นส่วนฐานของกะโหลกศีรษะที่อยู่ใกล้โครงสร้างทางประสาทและหลอดเลือดสำคัญ มีความเกี่ยวข้องทั้งในด้านคลินิกและพัฒนาการ แต่มีข้อมูลจำกัดในประชากรเอเชียตะวันออกเฉียงใต้ การศึกษานี้จึงตรวจสอบความชุกและลักษณะมอร์โฟเมตริกของความแปรผันต่าง ๆ ของกระดูก clivus ในกะโหลกศีรษะแห้งจากประชากรภาคตะวันออกเฉียงเหนือของไทยจำนวน 304 ซาก โดยประเมินรูปร่าง ขนาด ความสูง และความกว้างของความแปรผันที่พบมากที่สุด เช่น ร่อง fossa navicularis magna และตุ่มที่ขอบด้านหน้าของ foramen magnum โดยพบความแปรผันเหล่านี้ในร้อยละ 12.5 ของตัวอย่าง ซึ่งเป็นข้อมูลพื้นฐานที่สำคัญสำหรับการตีความภาพถ่ายทางรังสีและการผ่าตัดฐานกะโหลกศีรษะในบริบททางคลินิกและกายวิภาคศาสตร์

Abstract

Purpose

Anatomical variations of the clivus may have clinical and embryological relevance but remain underexplored in specific populations. This study investigated the prevalence and morphometric characteristics of clival anatomical variants in dried human skulls from the Northeastern Thai population.

Methods

Three hundred four dried human skulls (282 males, 22 females) were examined macroscopically. Variations of the clivus were identified and recorded by two independent observers, with discrepancies resolved by consensus. Descriptive statistics were used to report prevalence. Morphometric measurements (height and breadth) were taken for applicable variants using a digital caliper. Chi-square tests assessed sex differences.

Results

Fossa navicularis magna and tubercle at the anterior margin of the foramen magnum were the most prevalent variants, each observed in 38 skulls (12.5%). The basilar processes were found in 12 skulls (3.9%), condylus tertius in 5 skulls (1.6%), and prebasioccipital arch in 1 skull (0.3%). These variations showed variable morphometry. Fossa navicularis magna averaged 5.1 mm in height and 4.1 mm in breadth; tubercle at the anterior margin of the foramen magnum, 2.7 mm in breadth; basilar processes, 3.9 mm in height; condylus tertius, 6.3 mm in height and 6.9 mm in breadth; and prebasioccipital arch, 10.5 mm in height and 15.5 mm in breadth. No significant sex differences were found for fossa navicularis magna (p = 0.40), tubercle at the anterior margin of foramen magnum (p = 0.61) or basilar process (p = 0.67).

Conclusion

Clival variants, especially fossa navicularis magna and anterior tubercles, were found in 12.5% of skulls in this first study on Northeastern Thais. These may impact imaging interpretation and skull base surgery, reiterating the need for anatomical awareness in clinical contexts.

หลอดเลือดสมอง เช่น หลอดเลือดสมองส่วนหน้า (anterior cerebral artery), ส่วนกลาง (middle cerebral artery), และส่วนหลัง (posterior cerebral artery) มีบทบาทสำคัญในการลำเลียงเลือดไปยังสมองส่วนต่าง ๆ โดยเฉพาะในประชากรไทย การศึกษานี้มีเป้าหมายเพื่อวิเคราะห์ลักษณะทางมิติ เช่น เส้นผ่านศูนย์กลางและความยาวของหลอดเลือดเหล่านี้ โดยใช้เทคนิค digital subtraction angiography ซึ่งเป็นมาตรฐานทองคำสำหรับการวิเคราะห์หลอดเลือด พบว่ามีความแตกต่างทางเพศในเส้นผ่านศูนย์กลางและความยาว โดยเพศชายมีขนาดใหญ่กว่า ข้อมูลนี้สำคัญต่อการปรับปรุงการวางแผนการผ่าตัดและการออกแบบอุปกรณ์ทางการแพทย์ที่เหมาะสมสำหรับประชากรไทย

Abstract

Purpose

The main purpose of this study was to investigate the dimensions of cerebral arteries in the Thai population using digital subtraction angiography (DSA), with a focus on the anterior cerebral artery (ACA), middle cerebral artery (MCA), and posterior cerebral artery (PCA). The research aimed to explore anatomical variations in diameters and lengths of these arteries across different sexes and age groups, which are crucial for cerebrovascular interventions.

Methods

This retrospective study measured the diameters and lengths of the ACA, MCA, and PCA in 177 Thai patients with an average age of 47.6 years (range: 11–82 years) with normal cerebral angiograms. Digital subtraction angiography (DSA) was used for the measurements.

Results

The study found significant sex-based differences in the mean diameters of the left ACA (males: 2.12 ± 0.28 mm, females: 1.92 ± 0.20 mm; p < 0.01), right MCA (males: 2.50 ± 0.25 mm, females: 2.31 ± 0.17 mm; p < 0.01), and left MCA (males: 2.44 ± 0.19 mm, females: 2.30 ± 0.17 mm; p < 0.01) with males exhibiting larger diameters. The right ACA length was significantly longer in males (15.46 ± 1.74 mm) compared to females (13.98 ± 1.92 mm; p < 0.01). While no significant age-related differences were observed in diameters, a significant increase in length with age was noted for the left ACA in the ≥ 60 years group (13.61 ± 1.64 mm) compared to the < 60 years group (12.63 ± 1.36 mm; p < 0.01). Among others, significant correlations were found between the diameters of the left ACA and right MCA (r = 0.699; p < 0.01), and a strong correlation between left ACA length and left PCA diameter (r = 0.975; p < 0.01).

Conclusion

The findings provide invaluable data for tailoring neurosurgical approaches and designing angiographic equipment for the Thai population, emphasizing the importance of considering anatomical variations in clinical practice. These results highlight the necessity for personalized medical care based on anatomical differences to improve cerebrovascular intervention outcomes.

งานวิจัยฉบับนี้ศึกษาสัณฐานของกระดูกสันหลังส่วนคอชิ้นแรกหรือ atlas และศึกษาผลของเพศต่อขนาดและสัณฐานของกระดูกชิ้นนี้ด้วยปัญญาประดิษฐ์ งานวิจัยฉบับนี้ใช้กระดูกสันหลังส่วนคอชิ้นแรกจำนวนทั้งสิ้น 104 ชิ้น ซึ่งประกอบด้วยกระดูกจากผู้บริจาคร่างกายเพศชาย 54 ชิ้น และเพศหญิง 50 ชิ้น เพื่อวัดความยาวของ 12 พารามิเตอร์ (ระยะทาง a ถึง l) จากจุดสังเกตทางกายวิภาคจำนวน 23 จุด ผลการวิจัยพบว่า 8 จาก 12 พารามิเตอร์ มีความยาวที่มากกว่าในเพศชายอย่างมีนัยสำคัญทางสถิติ นอกจากนี้ยังนำความยาวของพารามิเตอร์เหล่านี้มาวิเคราะห์ด้วยปัญญาประดิษฐ์เพื่อจำแนกเพศ ผลการทดลองแสดงให้เห็นว่าโมเดลปัญญาประดิษฐ์ สามารถจำแนกเพศโดยการวิเคราะห์สัณฐานของกระดูก atlas โดยมีความแม่นยำที่ร้อยละ 82.6

Abstract

Purpose

The uppermost segment of the cervical vertebra or atlas (C1) is a critically important anatomical structure, housing the medulla oblongata and containing the grooves for the C1 spinal nerve and the vertebral vessels. Variations of the C1 vertebra can affect upper spine stability, and morphometric parameters have been reported to differ by population. However, there are few data regarding these parameters in Thais. The use of this bone to predict sex and age has never been reported.

Methods

This study aimed to examine C1 morphometry and determine its ability to predict sex. Twelve diameter parameters were taken from the C1 vertebrae of identified skeletons (n = 104, males [n, 54], females [n, 50]). Correlation analysis was also performed for sex and age, which were predicted using machine learning algorithms.

Results

The results showed that 8 of the 12 measured parameters were significantly longer in the male atlas (p < 0.05), while the remaining 4 (distance between both medial-most edges of the transverse foramen, transverse dimension of the superior articular surface, frontal plane passing through the canal’s midpoint, and anteroposterior dimension of the inferior articular surface) did not differ significantly by sex. There was no statistically significant difference in these parameters on the lateral side. The decision stump classifier was trained on C1 parameters, and the resulting model could predict sex with 82.6% accuracy (root mean square error = 0.38).

Conclusion

Assertation of the morphometric parameters of the atlas is important for preoperative assessment, especially for the treatment of atlas dislocation. Our findings also highlighted the potential use of atlas measurements for sex prediction.
