Pancreas divisum (PD) เป็นความผิดปกติแต่กำเนิดที่พบบ่อยที่สุดของระบบท่อตับอ่อน เกิดจากการที่ท่อตับอ่อนด้านหลังและด้านหน้าไม่หลอมรวมกันอย่างสมบูรณ์ในช่วงพัฒนาการของตัวอ่อน แม้ว่าจะมีการศึกษามาเป็นเวลานาน แต่ความชุกที่แท้จริงของภาวะนี้และความสัมพันธ์กับโรคตับอ่อน โดยเฉพาะตับอ่อนอักเสบ ยังคงเป็นประเด็นที่มีความขัดแย้งในวรรณกรรมทางการแพทย์ เนื่องจากรายงานก่อนหน้านี้ให้ผลลัพธ์ที่แตกต่างกันอย่างมาก และมักอาศัยข้อมูลจากผู้ป่วยที่ได้รับการตรวจ ERCP หรือ MRCP ซึ่งอาจเกิดอคติจากการคัดเลือกผู้ป่วย งานวิจัยนี้จึงดำเนินการทบทวนวรรณกรรมอย่างเป็นระบบและวิเคราะห์อภิมานจาก 117 การศึกษา ครอบคลุมประชากร 193,672 ราย เพื่อประเมินความชุกของ pancreas divisum ในระดับโลก รวมถึงศึกษาความสัมพันธ์ระหว่างภาวะดังกล่าวกับโรคตับอ่อน ผลการศึกษาช่วยสร้างฐานข้อมูลเชิงประจักษ์ที่มีความน่าเชื่อถือมากขึ้นเกี่ยวกับความสำคัญทางคลินิกของ pancreas divisum และเป็นประโยชน์ต่อการวินิจฉัย การประเมินความเสี่ยง และการวางแผนการรักษาผู้ป่วยโรคตับอ่อนในอนาคต

Abstract

Background and Objectives: Pancreas divisum (PD) is the most common congenital anomaly of the pancreatic ductal system and has been suggested to contribute to pancreatic pathology. However, its true prevalence and relationship with pancreatic diseases remain debated. This systematic review and meta-analysis aimed to estimate the global prevalence of PD and evaluate its association with pancreatic disease. 

Materials and Methods: A comprehensive search of Google Scholar, Scopus, and PubMed was conducted to identify studies reporting the prevalence of PD across all populations and diagnostic modalities. Pooled prevalence estimates were calculated using a random-effects model. Between-study heterogeneity was assessed using the I2 statistic, and publication bias was evaluated using Egger’s test. 

Results: A total of 117 studies comprising 193,672 subjects were included. The pooled global prevalence of PD was 11.1% (95% CI: 8.0–14.2%) with substantial heterogeneity (I2 = 99.96%). PD prevalence was higher among individuals with pancreatic disease (18.7%) compared with cadaveric/autopsy studies (8.8%), healthy individuals (5.6%), and consecutive patients (4.7%). Both complete and incomplete PD were more common in subjects with pancreatic diseases. Among PD subtypes, type I was the most prevalent. Egger’s test demonstrated significant publication bias (p < 0.01). 

Conclusions: PD affects approximately one in ten individuals worldwide and appears more prevalent in patients with pancreatic diseases. However, this finding should be interpreted with caution due to potential selection bias from predominantly ERCP-based studies.

Abstract

Ulnar neuropathy due to compression at the cubital tunnel is common. However, our understanding of the relationships between this type of nerve compression and the variant anconeus epitrochlearis muscle (AEM) is poorly understood. Therefore, the present anatomical study was performed to better elucidate these relationships. In 162 adult cadavers (324 sides), the roof of the cubital tunnel was dissected. The prevalence and gross anatomy of the AEM were documented. Histological and microCT analyses were performed on selected specimens to evaluate the microanatomy and radiological findings in relation to the underlying nerve, soft tissues, and bone. Additionally, with the range of motion of the elbow and artificial contraction of the AEM, the effects of the AEM on the ulnar nerve were observed. AEMs were identified on 32 (10%) of the sides. Histologically, a connective tissue connection between the AEM and ulnar nerve was found in all specimens, and a direct connection between the AEM and the underlying joint capsule was found in most specimens. No grossly visible compression of the ulnar nerve by the overlying AEM was observed with flexion or extension of the elbow. With artificial contraction of the AEM, the ulnar nerve was found to move out of the depths of the ulnar groove in roughly half of the sides. Previously unreported relationships between Osborne’s ligament, the AEM, and the underlying ulnar nerve were found. These findings will improve our understanding of the relationship between these structures and the ulnar nerve at the elbow. During surgery in this area, care should be taken to avoid injuring the ulnar nerve when applying traction to the AEM.

การแปรผันทางกายวิภาคของฐานกะโหลกศีรษะส่วน clivus เป็นข้อมูลสำคัญสำหรับการวินิจฉัยทางรังสีและการผ่าตัดศีรษะ–คอ เนื่องจากตำแหน่งที่อยู่ใกล้กับโครงสร้างประสาทและหลอดเลือดสำคัญ แต่มีข้อมูลเกี่ยวกับความแปรผันเหล่านี้ในประชากรไทยค่อนข้างจำกัด งานวิจัยนี้ใช้ภาพถ่ายเอกซเรย์คอมพิวเตอร์จากคนไทยมากกว่า 400 รายเพื่อประเมินความชุกและลักษณะของความแปรผันหลายแบบ เช่น ร่อง fossa navicularis magna ช่อง canalis basilaris medianus และตุ่มที่ขอบ foramen magnum ผลการศึกษาพบว่าความแปรผันเหล่านี้พบได้ค่อนข้างบ่อยที่สุดคือร่อง fossa navicularis magna ซึ่งมีความสำคัญต่อการตีความภาพรังสีให้ถูกต้องและช่วยให้แพทย์สามารถวางแผนการรักษาหรือการผ่าตัดได้อย่างปลอดภัยมากขึ้นในผู้ป่วยชาวไทย

Abstract

The clivus is an important landmark in neurosurgery and radiology due to its proximity to critical neurovascular structures. Anatomical variations of the clivus are diverse and may mimic pathological lesions on imaging. However, data on their prevalence in Southeast Asian populations are limited. This study investigated the prevalence and morphology of clival anatomical variations in the Thai population using computed tomography (CT). A total of 429 cranial CT scans (233 males and 196 females; mean age 39.5 ± 15.4 years) from patients without cranial abnormalities were retrospectively analyzed. The CT images were examined for fossa navicularis magna (FNM), basilar processes (BP), canalis basilaris medianus (CBM), tubercle at the anterior margin of the foramen magnum (TFM), condylus tertius (CT), and arcus praebasiooccipitalis (AP). FNM was the most common variation, identified in 107 cases (24.9%), followed by CBM (9.6%), BP (7.2%), and TFM (4.9%). CT (1.6%) and AP (0.5%) were rare. The CBM appeared as superior recess (5.1%), channel (2.6%), and inferior recess (2.3%) types. No significant sex differences were found for any variant (p > 0.05). These findings indicate that clival variations are relatively common in the Thai population, with FNM being the most prevalent. Knowledge of these variations is essential for accurate radiological interpretation and for planning neurosurgical procedures involving the skull base. Population-specific data such as these are crucial to improve diagnostic accuracy and surgical safety among Thai individuals.

ช่องว่างกลางคอนดิลาร์ (intermediate condylar canal) เป็นช่องเล็ก ๆ บนฐานกะโหลกศีรษะที่อยู่ใน fossa condyloid ด้านข้างของ condyle หลังศีรษะและด้านหลัง–ด้านกลางของ foramen jugulare โครงสร้างนี้มีการกล่าวถึงในตำราน้อยมากและยังไม่มีการศึกษาทางรังสีอย่างละเอียดมาก่อน งานวิจัยนี้จึงตรวจสอบการมีอยู่ของช่องว่างกลางคอนดิลาร์ในชุดข้อมูลการสแกนด้วยเอกซเรย์คอมพิวเตอร์จากอาสาสมัครชาวอินเดียจำนวน 490 ราย (820 ด้าน) พบช่องนี้ในร้อยละ 10.5 ของผู้ตรวจและร้อยละ 6 ของด้านกะโหลก พร้อมทั้งพบอุบัติการณ์แบบสองข้างและหนึ่งข้าง ซึ่งมีความสำคัญต่อศัลยแพทย์ฐานกะโหลกศีรษะเนื่องจากเป็นบริเวณที่เชื่อมต่อกับแนวทางผ่าตัดหลายแนว งานนี้เป็นการศึกษาพื้นฐานด้านรังสีวิทยาที่เปิดทางให้มีการศึกษาต่อในตัวอย่างกะโหลกจริงจากประชากรต่างเชื้อชาติได้ในอนาคต

Abstract


Intermediate condylar canal is a skull base canal in the condyloid fossa, located laterally to the occipital condyles and posteromedially to the jugular foramen. Despite its presence in the human skull base, this structure is rarely mentioned in textbooks and has not previously been examined radiologically. This study aimed to investigate the prevalence of intermediate condylar canal in CQ500 dataset, an open-access computed tomography scans of Indian population. A total of 490 head and neck computed tomography scans were thoroughly investigated. The presence of the intermediate condylar canal was investigated by two observers. Our results indicated the intermediate condylar canal was found in 10.5% of subjects (43 out of 410 individuals) or 6% of sides (49 out of 820 sides). Bilateral prevalence and unilateral prevalence were 1.5% (6 subjects) and 9% (37 subjects), respectively. No significant difference was found between sides. The width of these canals was 2.0 ± 0.5 mm (1.1–3.5 mm range). It is important to surgeons to be aware and familiar of intermediate condylar canal as it is located at the crucial crossroad of various surgical skull base approaches. This study serves as the fundamental research and further studies could include the investigation of intermediate condylar canal in skeletal specimens of various ethnic origins and imaging study to examine the content of the canals.

Abstract


The sternalis muscle, a well-documented anatomical variation in the chest muscles, has garnered attention in anatomical research but remains relatively unfamiliar to clinicians and radiologists. This variation exhibits a wide array of descriptions and classifications in the literature, emphasizing its highly variable characteristics. This study presents a new variant of the sternalis muscle with seven muscle bellies in a 79-year-old male donor. Bilateral accessory heads of the sternocleidomastoid muscles gave rise to two superior heads. Furthermore, five additional heads originated from the pectoralis major fascia, with three on the left and two on the right, together having widths of 6.6 cm on the left and 5.3 cm on the right. Innervation of the inferior heads was provided by the intercostal nerves. The configuration of the sternalis muscle with seven heads found in this study is exceptionally distinctive and has never been reported. This unique anatomical variation, coupled with three-dimensional imaging using photogrammetry, offers valuable insights for clinicians, especially in the context of breast surgery.

ที่มาและความสำคัญ

การแปรผันทางกายวิภาคของกระดูก clivus ซึ่งเป็นส่วนฐานของกะโหลกศีรษะที่อยู่ใกล้โครงสร้างทางประสาทและหลอดเลือดสำคัญ มีความเกี่ยวข้องทั้งในด้านคลินิกและพัฒนาการ แต่มีข้อมูลจำกัดในประชากรเอเชียตะวันออกเฉียงใต้ การศึกษานี้จึงตรวจสอบความชุกและลักษณะมอร์โฟเมตริกของความแปรผันต่าง ๆ ของกระดูก clivus ในกะโหลกศีรษะแห้งจากประชากรภาคตะวันออกเฉียงเหนือของไทยจำนวน 304 ซาก โดยประเมินรูปร่าง ขนาด ความสูง และความกว้างของความแปรผันที่พบมากที่สุด เช่น ร่อง fossa navicularis magna และตุ่มที่ขอบด้านหน้าของ foramen magnum โดยพบความแปรผันเหล่านี้ในร้อยละ 12.5 ของตัวอย่าง ซึ่งเป็นข้อมูลพื้นฐานที่สำคัญสำหรับการตีความภาพถ่ายทางรังสีและการผ่าตัดฐานกะโหลกศีรษะในบริบททางคลินิกและกายวิภาคศาสตร์

Abstract

Purpose

Anatomical variations of the clivus may have clinical and embryological relevance but remain underexplored in specific populations. This study investigated the prevalence and morphometric characteristics of clival anatomical variants in dried human skulls from the Northeastern Thai population.

Methods

Three hundred four dried human skulls (282 males, 22 females) were examined macroscopically. Variations of the clivus were identified and recorded by two independent observers, with discrepancies resolved by consensus. Descriptive statistics were used to report prevalence. Morphometric measurements (height and breadth) were taken for applicable variants using a digital caliper. Chi-square tests assessed sex differences.

Results

Fossa navicularis magna and tubercle at the anterior margin of the foramen magnum were the most prevalent variants, each observed in 38 skulls (12.5%). The basilar processes were found in 12 skulls (3.9%), condylus tertius in 5 skulls (1.6%), and prebasioccipital arch in 1 skull (0.3%). These variations showed variable morphometry. Fossa navicularis magna averaged 5.1 mm in height and 4.1 mm in breadth; tubercle at the anterior margin of the foramen magnum, 2.7 mm in breadth; basilar processes, 3.9 mm in height; condylus tertius, 6.3 mm in height and 6.9 mm in breadth; and prebasioccipital arch, 10.5 mm in height and 15.5 mm in breadth. No significant sex differences were found for fossa navicularis magna (p = 0.40), tubercle at the anterior margin of foramen magnum (p = 0.61) or basilar process (p = 0.67).

Conclusion

Clival variants, especially fossa navicularis magna and anterior tubercles, were found in 12.5% of skulls in this first study on Northeastern Thais. These may impact imaging interpretation and skull base surgery, reiterating the need for anatomical awareness in clinical contexts.

ในกรณีของการผ่าตัดบริเวณปลายล่างของกระดูกเรเดียส (distal radius) โดยเฉพาะในผู้ป่วยที่มีเนื้องอกหรือการบาดเจ็บรุนแรง อาจจำเป็นต้องตัดกระดูกออกทั้งหมด (en bloc resection) และใช้กระดูกอัลนา (ulna) มาทดแทนพร้อมการยึดตรึงด้วยแผ่นเหล็ก (plate fixation) ปัญหาสำคัญที่มักพบคือการคลายหรือหลุดของสกรูยึด ซึ่งส่งผลต่อความมั่นคงของการตรึงและผลลัพธ์ของการผ่าตัด งานวิจัยนี้จึงมีจุดมุ่งหมายเพื่อศึกษากลไกการหลุดของสกรูโดยใช้การวิเคราะห์องค์ประกอบจำกัด (finite element analysis) ร่วมกับการทดสอบทางชีวกลศาสตร์ในร่างอาจารย์ใหญ่ที่ดองด้วยวิธี Thiel ซึ่งรักษาสภาพความยืดหยุ่นของเนื้อเยื่อได้ดีและใกล้เคียงสภาพจริงมากกว่าศพแช่แข็ง ผลการศึกษาให้ข้อมูลสำคัญเกี่ยวกับตำแหน่งของสกรูที่มีแนวโน้มคลายตัวภายใต้แรงกดงอ และเสนอแนะแนวทางการวางสกรูและการเลือกใช้สกรูให้เหมาะสมกับตำแหน่งเสี่ยง เพื่อเพิ่มความมั่นคงในการตรึงแผ่นและลดภาวะแทรกซ้อนหลังผ่าตัด

Abstract

Fractures of the distal radius often require surgical intervention, with plate fixation being a standard stabilization method. Screw loosening and pull-out propose significant complications, necessitating comprehensive understanding of fixation stability factors. This study introduces a novel approach by the combination of finite element analysis (FEA) and experimental investigations on Thiel cadavers to evaluate screw pull-out behavior from plate fixation in en bloc distal radius resection with ulnar reconstruction. In comparison with previous investigations that used computational modeling or fresh-frozen cadaveric specimens, in the present research, FEA predictions specifically experimentally confirm the usage of Thiel cadavers, which better preserve soft tissue elasticity and hydration, thus more closely reflect in vivo conditions. Experimental set-up consisted of bending tests on cadavers and screw pull-out tests in Thiel-cadaveric radius specimens mimicking physiological conditions that induce the effects of screw pull-out. Finite element analysis and simulation were conducted using realistic clinical cases. Biomechanical test results indicated locking-plate deformation and screw loosening, particularly at locations closest to the ulnar bone gap. Torque measurements established various degrees of screw loosening, with the screws closest to the bone gap indicating maximum loosening. FEA demonstrated critical distributions of stresses in screws and locking plates, with good correlations to experimental findings. Screw pull-out force analysis showed vulnerability to loosening, particularly in the area of bone gaps, with findings consistent between biomechanical testing and FEA. This study offers valuable information on the surgical implications and biomechanical considerations of plate fixation for en bloc distal radius resection with ulnar reconstruction.

Abstract

Purpose

The nasal foramen is located in the nasal bone and for vessels passage to supply the nasal area. This project aimed to establish reliable references for the nasal foramina for future clinical applications.

Methods

The 72 dried skulls, 46 from the Division of Anatomy, University of Phayao, Thailand, and 26 from the Tulane University School of Medicine, USA, were collected and examined. The location, number, and sizes of nasal foramina were noted. The distances from each nasal foramen to the internasal suture, frontonasal suture, nasomaxillary suture, nasion, and rhinion were also recorded and used in the statistical analytical programs.

Results

The most common type of nasal foramen in all skulls was type II (one external opening) at 65.97%, followed by type I (no foramen opening) at 20.83%, type III (two external openings) at 11.11% and type IV at 2.08% (three external openings). Nasal foramen subtypes in many of the Thai and American skulls were type IIb and type IIa. The diameter of a connecting nasal foramen was significantly larger than that of a non-connecting. Results from embalmed confirmed the passage of the external nasal artery through the nasal cavity.

Conclusion

The study shows no significant difference in nasal foramen morphometry between Thai and American. It illustrates recent data on type and subtype classifications and the location of a vascular passage through the nasal foramen. This is the first study of NF variations and their respective classifications.

Abstract

The nasal bones are important bony parts of the external nose and maxillofacial scaffold. Generally, the nasal bone is a small quadrangular plate paired and attached to the frontonasal suture superiorly and the nasal septum internally. The nasal septum is symmetrically located and comprises the septal cartilage, the perpendicular plate of the ethmoid bone, and the vomer. Herein, we report the anatomy of single nasal bones (SNB) identified in two human skulls. Two adult human skulls were found to have SNB. In one, the nasal septum was bifid and hooked as it joined with the nasal bone superiorly. Understanding the morphological variations of the nasal bone and septum is important for maxillofacial surgeries and should be further clarified and studied. Although apparently very rare, a SNB, as seen in the present two cases, should be considered when interpreting imaging of the face.

ที่มาและความสำคัญ

สารสกัดน้ำจากดอกของต้นกะเพรา (Ocimum sanctum L.) มีสารประกอบฟีนอลิก เช่น กรดแกลลิก (gallic acid) และกรดคลอโรจีนิก ซึ่งมีบทบาทสำคัญในการปรับปรุงการทำงานของ tight junction ในเซลล์เยื่อบุลำไส้ การทำงานของ tight junction มีความสำคัญต่อการรักษาความสมบูรณ์ของเยื่อบุลำไส้ และลดการซึมผ่านของสารก่อการอักเสบ งานวิจัยนี้ชี้ให้เห็นว่า สารสกัดดังกล่าวช่วยกระตุ้นการรวมตัวของ tight junction ผ่านกลไกที่เกี่ยวข้องกับ CaMKKβ/AMPK/SIRT-1/ERK ซึ่งสามารถลดการซึมผ่านของ tight junction แบบ leak pathway และเพิ่มความต้านทานทางไฟฟ้าของเยื่อบุลำไส้ ผลการศึกษานี้มีศักยภาพในการพัฒนายาเพื่อรักษาโรคที่เกี่ยวข้องกับการเสียหายของ tight junction เช่น colitis และโรคลำไส้อักเสบ

Abstract

Intestinal tight junction disruption initiates pathogenesis of colitis and determines diseases progression. At present, there is no therapeutics that directly corrects intestinal tight junction disassembly. Here, we discovered that Ocimum sanctum L. flower aqueous extract (OSLF) and gallic acid, increased intestinal barrier function by suppressing tight junction-dependent leak pathway permeability after being disrupted by Ca2+ depletion method. Transepithelial electrical resistance (TER) measurement, western blot analysis, immunofluorescence staining, and molecular docking indicated that OSLF and gallic acid improved intestinal barrier function by inducing tight junction assembly and inhibiting leak pathway permeability in intestinal epithelial cell monolayers via CaMKK-β/AMPK/SIRT-1/ERK-dependent fashion. Therefore, Gallic acid represents a drug candidate for treating diseases associated impaired intestinal barrier function including colitis.
