ความหลวมคงเหลือ (residual laxity) หลังการผ่าตัดสร้างเอ็นไขว้หลังข้อเข่า (posterior cruciate ligament; PCL) แบบเส้นเอ็นเดี่ยว (single-bundle reconstruction) ยังคงเป็นปัญหาทางคลินิกที่อาจส่งผลกระทบต่อผลลัพธ์ด้านการใช้งานของข้อเข่าหลังผ่าตัดได้ การเสริมด้วยแถบเทปเย็บ (suture tape augmentation) ได้ถูกนำมาใช้เป็นเทคนิค internal brace เพื่อช่วยปกป้องเส้นเอ็นปลูกถ่าย (graft) และลดความหลวมของข้อเข่าในระยะแรกหลังการผ่าตัด
การศึกษานี้มีวัตถุประสงค์เพื่อประเมินว่าประสิทธิภาพการเสริมด้วย suture tape สามารถลดการเคลื่อนตัวไปด้านหลังของกระดูกหน้าแข้ง (posterior tibial translation; PTT) และการยืดตัวของเส้นเอ็นปลูกถ่าย (graft elongation) ภายใต้สภาวะการรับแรงซ้ำ ๆ (cyclic loading) ในการผ่าตัดสร้างเอ็นไขว้หลังข้อเข่าแบบเส้นเอ็นเดี่ยวได้

Abstract

Background

Residual laxity after single-bundle posterior cruciate ligament (PCL) reconstruction remains a clinical concern that may compromise functional outcomes. Suture tape augmentation has been introduced as an internal bracing technique to enhance graft protection and reduce early postoperative laxity. This study aimed to determine whether suture tape augmentation reduces posterior tibial translation (PTT) and graft elongation under cyclic loading in single-bundle PCL reconstruction.

Methods

Eight fresh-frozen human cadavers (16 knees) underwent bilateral single-bundle PCL reconstruction. One knee in each pair received suture tape augmentation, while the contralateral knee served as control. Posterior tibial translation was measured at 90° of flexion under four conditions: intact, after partial PCL transection, after reconstruction, and after 1000 loading cycles at 1 Hz. Graft elongation, diameter, and tibial tunnel size were recorded. Paired t-tests compared groups.

Results

No significant differences in PTT were found between groups in the intact, cut, or reconstructed states. After cyclic loading, the suture tape group showed lower PTT than controls (4.60 ± 2.24 mm vs 5.49 ± 1.52 mm, p = 0.06). The increase in PTT from reconstruction to post-cyclic loading was smaller with suture tape (1.09 ± 0.65 mm vs 1.57 ± 0.65 mm, p < 0.05). Graft elongation was similar, while graft diameter and tibial tunnel size were slightly larger with suture tape (p < 0.05).

Conclusion

Suture tape augmentation limited the increase in posterior tibial translation under cyclic loading, indicating improved dynamic stability and potential graft protection in single-bundle PCL reconstruction.

Abstract

The nasal bones are important bony parts of the external nose and maxillofacial scaffold. Generally, the nasal bone is a small quadrangular plate paired and attached to the frontonasal suture superiorly and the nasal septum internally. The nasal septum is symmetrically located and comprises the septal cartilage, the perpendicular plate of the ethmoid bone, and the vomer. Herein, we report the anatomy of single nasal bones (SNB) identified in two human skulls. Two adult human skulls were found to have SNB. In one, the nasal septum was bifid and hooked as it joined with the nasal bone superiorly. Understanding the morphological variations of the nasal bone and septum is important for maxillofacial surgeries and should be further clarified and studied. Although apparently very rare, a SNB, as seen in the present two cases, should be considered when interpreting imaging of the face.

ปุ่มกระดูกซิฟอยด์ (xiphoid process) เป็นส่วนที่เล็กที่สุดและมีความหลากหลายมากที่สุดของกระดูกหน้าอก (sternum) ซึ่งประกอบด้วยกระดูกและกระดูกอ่อน โดยในมนุษย์ กระดูกอ่อนบริเวณปุ่มกระดูกซิฟอยด์ถูกมองว่าเป็นความแปรผันทางกายวิภาค การศึกษานี้มีวัตถุประสงค์เพื่อสำรวจลักษณะทางกายวิภาคของปุ่มกระดูกซิฟอยด์ผ่านการตัดและตรวจด้วยกล้องจุลทรรศน์ พบว่ารูปแบบของปุ่มกระดูกซิฟอยด์สามารถจัดแบ่งได้เป็น 4 แบบตามสัดส่วนของกระดูกและกระดูกอ่อน ซึ่ง 85% ของปุ่มกระดูกซิฟอยด์มีส่วนประกอบของกระดูกและกระดูกอ่อน การศึกษานี้เน้นให้เห็นความสำคัญของการพิจารณากระดูกอ่อนเป็นส่วนหนึ่งของลักษณะทางกายวิภาคปกติ

Abstract

Background

The xiphoid process (XP) in animals such as sheep and rats are well known to have cartilage called xiphoidal cartilage (XC). In humans, the cartilage in the xiphoid process is considered an anatomical variant and is not well understood. The aim of this study was to investigate the morphology of the XP.

Methods

A total of twenty embalmed European descendant cadaveric sterna (aged 52 to 98 years) were used. Transilluminated XPs and midsagittal sections of XPs were used to examine the bone and cartilage. Subsequently, a sagittally-sectioned XP was harvested for histology and stained with Masson’s trichrome. The results of the transillumination and histological examinations were compared qualitatively.

Results

The dark area visible in transilluminated XPs was consistent with the bony part in the midsagittal XP sections, which contained bone marrow; the bright area was consistent with the cartilage part in the midsagittal XP sections. This was all demonstrated histologically. Most of the XPs (85%) had some portion of cartilage. The XP was classified into four types based on its proportions of bone and cartilage: Type I, no ossification (< 1/3 ossification) 45%; Type II, minor ossification (1/3 − 1/2 ossification) 20%; Type III, major ossification (1/2–2/3 ossification) 20%; Type IV, complete ossification (> 2/3 ossification) 15%. Most of the XPs (85%) had bone and cartilage, which could have been overlooked in studies using skeletons or CT.

Conclusion

Previous studies probably underestimated or overestimated the size of the XP. The XC needs to be considered as normal anatomy.
