ที่มาและความสำคัญ

การแปรผันทางกายวิภาคของฐานกะโหลกศีรษะส่วน clivus เป็นข้อมูลสำคัญสำหรับการวินิจฉัยทางรังสีและการผ่าตัดศีรษะ–คอ เนื่องจากตำแหน่งที่อยู่ใกล้กับโครงสร้างประสาทและหลอดเลือดสำคัญ แต่มีข้อมูลเกี่ยวกับความแปรผันเหล่านี้ในประชากรไทยค่อนข้างจำกัด งานวิจัยนี้ใช้ภาพถ่ายเอกซเรย์คอมพิวเตอร์จากคนไทยมากกว่า 400 รายเพื่อประเมินความชุกและลักษณะของความแปรผันหลายแบบ เช่น ร่อง fossa navicularis magna ช่อง canalis basilaris medianus และตุ่มที่ขอบ foramen magnum ผลการศึกษาพบว่าความแปรผันเหล่านี้พบได้ค่อนข้างบ่อยที่สุดคือร่อง fossa navicularis magna ซึ่งมีความสำคัญต่อการตีความภาพรังสีให้ถูกต้องและช่วยให้แพทย์สามารถวางแผนการรักษาหรือการผ่าตัดได้อย่างปลอดภัยมากขึ้นในผู้ป่วยชาวไทย

Abstract

The clivus is an important landmark in neurosurgery and radiology due to its proximity to critical neurovascular structures. Anatomical variations of the clivus are diverse and may mimic pathological lesions on imaging. However, data on their prevalence in Southeast Asian populations are limited. This study investigated the prevalence and morphology of clival anatomical variations in the Thai population using computed tomography (CT). A total of 429 cranial CT scans (233 males and 196 females; mean age 39.5 ± 15.4 years) from patients without cranial abnormalities were retrospectively analyzed. The CT images were examined for fossa navicularis magna (FNM), basilar processes (BP), canalis basilaris medianus (CBM), tubercle at the anterior margin of the foramen magnum (TFM), condylus tertius (CT), and arcus praebasiooccipitalis (AP). FNM was the most common variation, identified in 107 cases (24.9%), followed by CBM (9.6%), BP (7.2%), and TFM (4.9%). CT (1.6%) and AP (0.5%) were rare. The CBM appeared as superior recess (5.1%), channel (2.6%), and inferior recess (2.3%) types. No significant sex differences were found for any variant (p > 0.05). These findings indicate that clival variations are relatively common in the Thai population, with FNM being the most prevalent. Knowledge of these variations is essential for accurate radiological interpretation and for planning neurosurgical procedures involving the skull base. Population-specific data such as these are crucial to improve diagnostic accuracy and surgical safety among Thai individuals.

ช่องกระดูกขากรรไกร (mandibular canal) เป็นช่องที่สำคัญในการส่งผ่านเส้นประสาทและหลอดเลือดในขากรรไกรล่าง แต่รูปแบบความแปรผันของช่องกระดูกขากรรไกรยังมีรายงานไม่มากนัก การศึกษานี้รายงานความแปรผันทางกายวิภาคที่ไม่เคยอธิบายมาก่อนของช่องกระดูกขากรรไกรแบบไม่สมบูรณ์ทั้งสองข้างในกระดูกขากรรไกรจากซากศพชาย ด้วยการตรวจทางกายวิภาคและภาพเอกซเรย์คอมพิวเตอร์ จากนั้นใช้ชิ้นเนื้อจากตัวอ่อนมนุษย์เพื่อสร้างแบบจำลองสามมิติ วิเคราะห์ความสัมพันธ์ระหว่างกระดูกขากรรไกรที่กำลังพัฒนา เส้นประสาทขากรรไกรล่าง และต่อมน้ำลาย ผลการศึกษาสนับสนุนสมมติฐานว่า การก่อตัวของช่องที่ไม่สมบูรณ์อาจเกิดจากตำแหน่งของเส้นประสาทหรือการถูกกักขังของเนื้อเยื่อต่อมน้ำลายในระหว่างการพัฒนา ความแปรผันนี้มีความสำคัญต่อการตีความภาพทางรังสีและการวางแผนผ่าตัดขากรรไกรล่างในทางคลินิก

Abstract

Purpose

This study aims to report a previously undocumented anatomical variation of the mandible and investigate its developmental origin using embryological analysis.

Methods

Anatomical inspection and computed tomography were performed on a dried human mandible from a male cadaver of unknown age. Additionally, serial histological sections from a Carnegie Stage 23 human embryo were reconstructed in 3D model to examine the spatial relationships between the developing mandible and surrounding structures.

Results

The mandibular canal exhibited breaching of the medial wall into the lingual cortex on both sides. Computed tomography confirmed that the canals originated from the mandibular foramen and gave rise to the mental foramina at the medial third of the mandible. These defects were found along with a left-sided Stafne bone cavity. Embryological analysis revealed a close spatial proximity between the developing mandible, the inferior alveolar nerve, and the salivary glands, supporting the hypothesis that incomplete ossification could result from variant nerve positioning or glandular entrapment.

Conclusion

This study presents rare anatomical variations of the mandibular canal and their possible developmental mechanism. These findings have implications for radiological interpretation and surgical planning involving the mandible.

ช่องว่างกลางคอนดิลาร์ (intermediate condylar canal) เป็นช่องเล็ก ๆ บนฐานกะโหลกศีรษะที่อยู่ใน fossa condyloid ด้านข้างของ condyle หลังศีรษะและด้านหลัง–ด้านกลางของ foramen jugulare โครงสร้างนี้มีการกล่าวถึงในตำราน้อยมากและยังไม่มีการศึกษาทางรังสีอย่างละเอียดมาก่อน งานวิจัยนี้จึงตรวจสอบการมีอยู่ของช่องว่างกลางคอนดิลาร์ในชุดข้อมูลการสแกนด้วยเอกซเรย์คอมพิวเตอร์จากอาสาสมัครชาวอินเดียจำนวน 490 ราย (820 ด้าน) พบช่องนี้ในร้อยละ 10.5 ของผู้ตรวจและร้อยละ 6 ของด้านกะโหลก พร้อมทั้งพบอุบัติการณ์แบบสองข้างและหนึ่งข้าง ซึ่งมีความสำคัญต่อศัลยแพทย์ฐานกะโหลกศีรษะเนื่องจากเป็นบริเวณที่เชื่อมต่อกับแนวทางผ่าตัดหลายแนว งานนี้เป็นการศึกษาพื้นฐานด้านรังสีวิทยาที่เปิดทางให้มีการศึกษาต่อในตัวอย่างกะโหลกจริงจากประชากรต่างเชื้อชาติได้ในอนาคต

Abstract


Intermediate condylar canal is a skull base canal in the condyloid fossa, located laterally to the occipital condyles and posteromedially to the jugular foramen. Despite its presence in the human skull base, this structure is rarely mentioned in textbooks and has not previously been examined radiologically. This study aimed to investigate the prevalence of intermediate condylar canal in CQ500 dataset, an open-access computed tomography scans of Indian population. A total of 490 head and neck computed tomography scans were thoroughly investigated. The presence of the intermediate condylar canal was investigated by two observers. Our results indicated the intermediate condylar canal was found in 10.5% of subjects (43 out of 410 individuals) or 6% of sides (49 out of 820 sides). Bilateral prevalence and unilateral prevalence were 1.5% (6 subjects) and 9% (37 subjects), respectively. No significant difference was found between sides. The width of these canals was 2.0 ± 0.5 mm (1.1–3.5 mm range). It is important to surgeons to be aware and familiar of intermediate condylar canal as it is located at the crucial crossroad of various surgical skull base approaches. This study serves as the fundamental research and further studies could include the investigation of intermediate condylar canal in skeletal specimens of various ethnic origins and imaging study to examine the content of the canals.

Abstract


The sternalis muscle, a well-documented anatomical variation in the chest muscles, has garnered attention in anatomical research but remains relatively unfamiliar to clinicians and radiologists. This variation exhibits a wide array of descriptions and classifications in the literature, emphasizing its highly variable characteristics. This study presents a new variant of the sternalis muscle with seven muscle bellies in a 79-year-old male donor. Bilateral accessory heads of the sternocleidomastoid muscles gave rise to two superior heads. Furthermore, five additional heads originated from the pectoralis major fascia, with three on the left and two on the right, together having widths of 6.6 cm on the left and 5.3 cm on the right. Innervation of the inferior heads was provided by the intercostal nerves. The configuration of the sternalis muscle with seven heads found in this study is exceptionally distinctive and has never been reported. This unique anatomical variation, coupled with three-dimensional imaging using photogrammetry, offers valuable insights for clinicians, especially in the context of breast surgery.

รายงานนี้นำเสนอรูเปิดปล่อยเลือดที่ผิดปกติในกระดูกขมับของผู้ป่วยชายอายุ 64 ปี ซึ่งไม่ตรงกับรูเปิดที่อธิบายไว้ก่อนหน้านี้ การรู้จักรูปแบบความแปรผันเช่นนี้สำคัญเพื่อลดการตีความผิดพลาดในภาพถ่ายและป้องกันภาวะแทรกซ้อนในการผ่าตัดฐานกะโหลกศีรษะ

รูเปิดปล่อยเลือด (emissary foramen) เป็นช่องขนาดเล็กในกระดูกกะโหลกศีรษะที่เชื่อมต่อระบบหลอดเลือดดำภายในกะโหลกกับเครือข่ายหลอดเลือดดำภายนอก ทั้งนี้รูเปิดปล่อยเลือดที่พบบ่อยได้รับการอธิบายอย่างดีแล้ว แต่ความแปรผันที่ผิดปกติยังไม่ได้รับความสนใจเพียงพอ การศึกษานี้รายงานกรณีรูเปิดปล่อยเลือดที่ไม่ปกติในกระดูกข้างขมับของผู้ป่วยชายอายุ 64 ปีจากภาพถ่ายเอกซเรย์คอมพิวเตอร์ความละเอียดสูง พบช่องเปิดขนาด 3.15 มม. ที่แยกออกจากโพสท์เกลนอยด์ฟอราเมนและไม่พบในฝั่งตรงข้าม โดยตำแหน่งของรูเปิดนี้มีแนวโน้มแสดงการเชื่อมต่อหลอดเลือดดำจากพีทีรอยด์เวนัสเพล็กซัสสู่ช่องหลอดเลือดดำภายในกะโหลก การรับรู้รูเปิดที่ผิดปกติประเภทนี้มีความสำคัญต่อการตีความภาพทางการแพทย์ ป้องกันการวินิจฉัยผิดพลาด และเตรียมการผ่าตัดฐานกะโหลกศีรษะอย่างรอบคอบเพื่อหลีกเลี่ยงภาวะแทรกซ้อนเช่นเลือดออกโดยไม่คาดคิด

การแปรผันทางกายวิภาคของกระดูก clivus ซึ่งเป็นส่วนฐานของกะโหลกศีรษะที่อยู่ใกล้โครงสร้างทางประสาทและหลอดเลือดสำคัญ มีความเกี่ยวข้องทั้งในด้านคลินิกและพัฒนาการ แต่มีข้อมูลจำกัดในประชากรเอเชียตะวันออกเฉียงใต้ การศึกษานี้จึงตรวจสอบความชุกและลักษณะมอร์โฟเมตริกของความแปรผันต่าง ๆ ของกระดูก clivus ในกะโหลกศีรษะแห้งจากประชากรภาคตะวันออกเฉียงเหนือของไทยจำนวน 304 ซาก โดยประเมินรูปร่าง ขนาด ความสูง และความกว้างของความแปรผันที่พบมากที่สุด เช่น ร่อง fossa navicularis magna และตุ่มที่ขอบด้านหน้าของ foramen magnum โดยพบความแปรผันเหล่านี้ในร้อยละ 12.5 ของตัวอย่าง ซึ่งเป็นข้อมูลพื้นฐานที่สำคัญสำหรับการตีความภาพถ่ายทางรังสีและการผ่าตัดฐานกะโหลกศีรษะในบริบททางคลินิกและกายวิภาคศาสตร์

Abstract

Purpose

Anatomical variations of the clivus may have clinical and embryological relevance but remain underexplored in specific populations. This study investigated the prevalence and morphometric characteristics of clival anatomical variants in dried human skulls from the Northeastern Thai population.

Methods

Three hundred four dried human skulls (282 males, 22 females) were examined macroscopically. Variations of the clivus were identified and recorded by two independent observers, with discrepancies resolved by consensus. Descriptive statistics were used to report prevalence. Morphometric measurements (height and breadth) were taken for applicable variants using a digital caliper. Chi-square tests assessed sex differences.

Results

Fossa navicularis magna and tubercle at the anterior margin of the foramen magnum were the most prevalent variants, each observed in 38 skulls (12.5%). The basilar processes were found in 12 skulls (3.9%), condylus tertius in 5 skulls (1.6%), and prebasioccipital arch in 1 skull (0.3%). These variations showed variable morphometry. Fossa navicularis magna averaged 5.1 mm in height and 4.1 mm in breadth; tubercle at the anterior margin of the foramen magnum, 2.7 mm in breadth; basilar processes, 3.9 mm in height; condylus tertius, 6.3 mm in height and 6.9 mm in breadth; and prebasioccipital arch, 10.5 mm in height and 15.5 mm in breadth. No significant sex differences were found for fossa navicularis magna (p = 0.40), tubercle at the anterior margin of foramen magnum (p = 0.61) or basilar process (p = 0.67).

Conclusion

Clival variants, especially fossa navicularis magna and anterior tubercles, were found in 12.5% of skulls in this first study on Northeastern Thais. These may impact imaging interpretation and skull base surgery, reiterating the need for anatomical awareness in clinical contexts.

การสร้างและดัดแปลงอนุภาคโนด้าไวรัส MrNV-VLPs ให้มีความจำเพาะต่อกลุ่มน้ำตาลฟูโคสบนเปลือกหุ้ม คาดว่าจะสามารถยับยั้งการติดเชื้อไวรัสได้อย่างมีประสิทธิภาพ เนื่องโนด้าไวรัส ใช้กลุ่มน้ำตาลฟูโคสในการจับและเข้าสู่เซลล์ นอกจากนี้อนุภาคโนด้าไวรัสยังมีคุณสมบัติเด่นด้านความเสถียร และความแรงของการจับ (avidity) ที่สูง นอกจากนี้ยังสามารถนำมาใช้ประโยชน์ได้ง่าย เนื่องจากสามารถผลิต ปริมาณมาก เก็บรักษา และใช้งานได้ง่ายกว่าเลคทินหรือไกลแคนแบบดั้งเดิม

Abstract

Our previous studies demonstrated that Macrobrachium rosenbergii nodavirus (MrNV) infects and replicates in Sf9 insect cells, where fucosylated N- and O-glycans (HexNAc (Fuc)HexNAc-R or Fuc-LacdiNAc) serve as essential binding molecules for viral attachment. Additionally, we showed that the virus-like particles of the virus itself (MrNV-VLPs) were able to bind to the white-tail disease (WTD)-associated tissues and infect Sf9 cells; and these processes were significantly reduced by removing 27 amino acids from the C-terminus of the protruding (P) domain. In this study, we demonstrated that icosahedral particles could still form following the truncation of the P-domain at sites adjacent to known calcium-binding domains (CBDs) present in the S-shell domain in the MrNV capsid protein. We further showed that icosahedral particles could still form after truncation of the P-domain near calcium-binding sites in the S-domain. This generated smaller but intact particles lacking protrusions, designated V250-MrNV-VLPs. The particles with their intact S-shells enabled our complete replacement of the original MrNV capsid protein P-domains with the fucose-binding carbohydrate-recognition domain (CRD) of CLEC17A lectin (Prolectin), successfully creating CLEC17A/CRD-MrNV-VLPs. The chimeric CLEC17A/CRD-MrNV-VLPs were stable icosahedral particles with P-domains structurally distinct from both the smooth V250-MrNV-VLPs and the blade-like domains of wild-type MrNV-VLPs. Furthermore, the chimeric VLPs presented specific binding activities towards immobilized fucosylated glycoconjugates and Sf9 cell protein lysates. Notably, CLEC17A/CRD-MrNV-VLPs were able to reduce MrNV binding and infection in Sf9 cells. Therefore, this study demonstrated the potential for CLEC17A/CRD-MrNV-VLPs as particles that can effectively bind to fucosylated glycans, and their potential development as nanoparticles that can significantly reduce the level of infection by MrNV in susceptible cells.

ที่มาและความสำคัญ

โรคพยาธิใบไม้ตับ (Fasciola gigantica) เป็นปัญหาสำคัญในสัตว์เคี้ยวเอื้อง โดยเฉพาะในแพะและโค ก่อให้เกิดการสูญเสียทางเศรษฐกิจจากการลดน้ำหนัก โตช้า และลดประสิทธิภาพการผลิต ปัจจุบันการควบคุมโรคยังพึ่งพายาถ่ายพยาธิเป็นหลัก ซึ่งมีความเสี่ยงต่อการดื้อยาเพิ่มขึ้นอย่างต่อเนื่อง จึงมีความจำเป็นเร่งด่วนในการพัฒนาวิธีป้องกันใหม่ เช่น วัคซีน การศึกษานี้จึงศึกษาศักยภาพของโปรตีน cathepsin L1H ในการลดการติดเชื้อพยาธิ และเป็นก้าวสำคัญสู่การพัฒนาวัคซีนที่มีประสิทธิภาพสำหรับควบคุมโรคพยาธิใบไม้ตับในแพะในอนาคต

Abstract

Cathepsin L1H has been identified as a potential vaccine candidate against fasciolosis. The present study was conducted to evaluate the immunoprophylactic efficacy of recombinant proFgCatL1H (rproFgCatL1H) antigen in goats subsequently challenged with the parasite. Immunization of goats with 200 µg of rproFgCatL1H formulated in Montanide™ ISA 61 VG conferred statistically significant reduction in worm recovery compared with both the infected control and adjuvant control groups, corresponding to reductions of 37.55 % and 34.07 %, respectively (p < 0.05). A negative correlation was observed between worm burden reduction and serum IgG levels (OD₄₅₀), measured both at the time of infection and at the study endpoint, although the association was not statistically significant. Scanning Electron Microscopy (SEM) analysis of adult F. gigantica recovered from vaccinated goats demonstrated pronounced tegumental damage, characterized by extensive swelling, surface erosion, and the presence of cell-like structures adhering to the tegumental surface. These findings suggest that the recombinant protein rproFgCatL1H holds promise as a potential vaccine candidate for the control of fasciolosis in goats.

สาหร่ายแดง (Rhodophyta) เป็นทรัพยากรทางทะเลที่สำคัญ จากงานวิจัยศักยภาพของสาหร่ายแดงและสารออกฤทธิ์ทางชีวภาพสำคัญ ได้แก่สารพอลิแซ็กคาไรด์ที่เป็นองค์ประกอบหลักเช่น คาร์ราเจแนน อะการ์ และซัลเฟตกาแลคแตน พบมีคุณสมบัติส่งเสริมการเติบโต สุขภาพ และภูมิคุ้มกันของสัตว์น้ำ งานวิจัยที่ผ่านมาได้รายงานการประยุกต์ใช้สาหร่ายแดงในหลายรูปแบบ เช่นเป็นส่วนผสมในอาหารสัตว์ การทดแทนปลาป่นบางส่วน หรือการเป็นสารกระตุ้นภูมิคุ้มกัน ต้านทานโรค สอดคล้องกับแนวทางการพัฒนาการเพาะเลี้ยงสัตว์น้ำอย่างยั่งยืน บทความรีวิวฉบับนี้ได้รวบรวมองค์ความรู้ที่กระจัดกระจาย วิเคราะห์ และนำเสนอข้อมูลเชิงลึก ชี้ให้เห็นข้อได้เปรียบและข้อจำกัด ตลอดจนระบุทิศทางการวิจัยในอนาคตเพื่อการประยุกต์ใช้ประโยชน์จากสาหร่ายที่เป็นทรัพยากรทางทะเลได้อย่างมีประสิทธิภาพมากขึ้น

Abstract

Red seaweeds (Rhodophyta) and their polysaccharides represent promising, sustainable solutions for boosting aquaculture productivity. Rich in bioactive compounds such as carrageenan, agar, and sulfated galactans, these seaweeds have demonstrated the ability to improve growth, health, and disease resistance in various aquaculture species. This review examines the applications, benefits, and limitations of red seaweeds in aquaculture, with particular attention to their roles as feed binders, partial fishmeal replacements, feed supplements, and immunostimulants. Species such as Gracilaria, Kappaphycus, and Porphyra have been shown to enhance immune responses, promote gut health, and mitigate nutrient pollution in aquaculture systems. Despite these advantages, challenges remain, including high cultivation costs, variability in bioactivity, and supply chain constraints. This review highlights the growing potential of red seaweeds for sustainable aquaculture while identifying research needs related to dosage optimization, delivery strategies, and elucidating the molecular mechanisms of their effects. Supplementation with red seaweeds aligns with the United Nations’ Sustainable Development Goals, offering an eco-friendly alternative to antibiotics with the potential to yield both environmental and economic benefits in aquaculture.

โครงการ Soft cadaver Workshop เป็นความร่วมมือกันระหว่างภาควิชากายวิภาคศาสตร์ คณะวิทยาศาสตร์ และ คณะแพทยศาสตร์ โรงพยาบาลรามาธิบดี หรือสถาบันการแพทย์อื่น ๆ เพื่อเพิ่มพูนทักษะการทำหัตถการทางการแพทย์ต่าง ๆ เช่น การฝึกผ่าตัด ให้กับ นศพ.ชั้นคลินิก (ปี5-6) แพทย์ประจำบ้าน และแพทย์ประจำบ้านต่อยอดได้ฝึกทำหัตถการก่อนผ่าตัดรักษาคนไข้จริง โดยแพทย์ผู้ร่วมงานจะได้รับการฝึกฝนจากคณาจารย์ศัลยแพทย์ผู้เชี่ยวชาญ ภาควิชากายวิภาคศาสตร์จึงมีการเตรียมร่างอาจารย์ใหญ่เสมือนจริงแบบนิ่ม (Soft Cadaver) เพื่อสนับสนุนการจัดการเรียนการสอน และการฝึกฝึกอบรมผ่าตัดเสมือนจริงแก่แพทย์เพื่อนำไปใช้ประโยชน์ในภายภาคหน้าต่อไป

วัตถุประสงค์

เพื่อให้นักศึกษาแพทย์ชั้นคลินิก (ชั้นปี5-6) แพทย์ประจำบ้าน และ แพทย์ประจำบ้านต่อยอดของภาควิชาศัลยศาสตร์ได้ฝึกหัตถการผ่าตัดการแพทย์ เพื่อเพิ่มพูนทักษะความชำนาญในการผ่าตัดรักษาคนไข้ต่อไป
